New Patients

What should I do before my first visit?

New patients can call our office and complete our contact form to schedule your first appointment.

If you have any questions about whether or not you need referrals, please ask our office staff or your insurance provider at least two days prior to coming to the office.

Please remember to bring the following to your first appointment:

  • Insurance card
  • Picture I.D.
  • Any necessary referrals
  • A list of your current medications
  • Copies of any blood tests or X-rays that may be related to the reason you are seeing us
  • You may wish to print out our new patient forms prior to your visit

Please Note: Since we may be skin testing you, try to avoid taking antihistamines (allergy pills) for 5 to 7 days before the visit. If you have children coming in, you may wish to provide us with any medical forms needed for school or camp.

We REQUIRE 24 hours advance notice for cancellations on ALL appointments or we charge a $50 no show/late cancellation fee.

What should I expect at my first visit?

Your first visit will take about one hour.

During the visit we will take a careful medical history, physical examination, and possibly perform skin testing. If you have breathing problems, we may also perform a lung function test.

We will begin the process of treating your condition with the proper medications, advising you on avoidance of possible allergens, and discussing whether allergy injections are right for you.

You will be able to ask any questions so that you have a clear understanding of your diagnosis and treatment plan.
